High Priority – Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit. Cut cabbage 50°. Item placed on cook line at 11 per manager. Item moved to walk in cooler. Chicken 58°. On rack at end of cook line. Two other pans of chicken on same rack held at room temperature overnight. Observed mold like growth on one pan. Manager immediately discarded. Unable to get temperature.

High Priority – Raw animal food stored over ready-to-eat food. In walk in cooler Raw shrimp above cake. Raw pork stored above eggrolls. In walk in freezer, products not commercially packaged. Chicken over beef. Discussed proper storage requirements with manager.

High Priority – Roach activity present as evidenced by live roaches found. 4 live roaches observed inside container for plastic wrap. Manager discarded container. 1 live roach observed on prep table with plastic wrap. 1 live roach observed on side of oil container stored on floor under wok station. 1 live roach observed on sealed bag of cornstarch stored on dry storage shelves across from men's restroom. Manager killed all observed roaches. Manager also stated that pest control has been conducted and is scheduled to return at 2:00.

Lid is hanging over bowls/dish ware. Approximately 15 dead roaches between wall and pipe on chemical sanitizer dispenser at three compartment sink.
