Basic – A minimum of one bathroom facility is not available for public use. Observed women's and Mens bathroom closed off, the general manager and I tried to flush toilets in both bathrooms and water started to come up from base of both toilets and run into drain on the floor. The floor drain started to flood and have standing water. There is no other bathroom available for use. **Warning**

Basic – Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed ceiling tiles above cooks line with food debris and dust.

Basic – Exterior door has a gap at the threshold that opens to the outside. Gap at back door that opens to outside in kitchen.

Basic – Floor soiled/has accumulation of debris. Observed floor in kitchen soiled with grease and debris on cooks line by fryers.
