Submit a license application and payment to DBPR/Bureau of Central Intake Unit, 2601 Blair Stone Rd., Tallahassee, FL 32399-0783. Plans submitted 02/05/2025 and Plan Review stated they were working on plans from 02/03/2025 at time of callback. **Time Extended**

Must submit plans and plan review application to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Plans must be submitted AND approved within 60 days. Observed hand wash sink next to three compartment sink per old establishment plans has been moved to exterior of walk-in cooler next to soda machine and plans not updated. **Warning** – From follow-up inspection 2025-02-03: At callback observed establishment with updated plans but did not send to plan review. Establishment began sending plans to plan review. **Time Extended** – From follow-up inspection 2025-02-12: At callback observed updated plans submitted to Plan Review on 02/05/2025. Inspector contacted Plan Review and they stated that they were working on plans from 02/03/2025 and have not had ample time to go over plans for following days. **Time Extended**
