High Priority – – From initial inspection : High Priority – Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. At time of inspection 4 dead flies at self service kiosk, 2 dead flies at counter register, 1 dead fly on self service ketchup dispenser. Observed 1 live fly in the prep area, employee attempted to kill and remove, operator believes fly landed on fly trap. On window sill of dining room behind booth 20 dead flies. Employee swept and discarded all dead flies. **Corrective Action Taken** **Admin Complaint** – From follow-up inspection 2025-06-18: At time of follow up inspection 6 live flies observed through kitchen area. Two flies observed by rear exterior door landing on racks of bagged bread. Two flies observed on cook line grill area and reach in cooler landing on hot holding equipment. Two flies observed on front cook line area and counter opposite cash register by ice cream equipment. Observed one live fly in dining room area. One dead fly observed on counter of reach in cooler and multiple dead flies caught in fly tape in prep area with build up of flies. **Admin Complaint** – From follow-up inspection 2025-06-19: At time of follow up inspection 10 live flies observed in kitchen area of establishment. Observed 1 live fly at the front counter on the entry door, 1 live fly on the front counter cookie container, 2 live flies in back prep room area on bread rack, operator was able to kill 1 fly, 5 live flies on cook line over flat top grill area landing on open package of bread and hot holding equipment and single service deli paper. Operator was able to kill 3 flys and discarded the deli paper from the cook line. Observed 1 live fly in restroom and 2 live flies in dining room on window. Observed 1 dead fly in water of three compartment sink, operator drained the dish water. Multiple dead flies observed in fly sticky tape in prep area.
