Observed two live flying insects in main kitchen near back door. Observed approximately 20 live flying insects trapped in control devices in between prep kitchen and main kitchen. **Warning** – From follow-up inspection 2025-06-25: Observed one flying insect in dish pit. Observed approximately 50 flying insects around fly trap outside restaurant near back door. **Admin Complaint**

Person in charge immediately called plumber and ceased use of dish machine. Observed large pool of waste water pooling near prep kitchen and ice machine creating a sanitary nuisance and attracting flying insects. **Warning** – From follow-up inspection 2025-06-25: Observed dish machine plumbing fixed. Observed double sink next to dish machine leaking. Observed waste water puddle outback of restaurant still there. Person in charge had plumber check out drain. Observed clear water flowing from floor drain in outback concrete pad. Water flow rate stopped during inspection person in charge called plumber. **Admin Complaint**
