03A-02-5High Priority – Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In the front counter observed containers of butter balls dairy based butter (64F – Cold Holding). Operator stated butter had been out for one hour and 45 minutes. Discussed with operator time temperature control. Provided operator with Time as a public health control form. Operator began filling out form and placed unused butter back into a cold holding unit. **Corrective Action Taken**

02D-03-4Basic – Label on a food item prepared and packaged onsite for customer self-service does not include product identity/description, date product was packaged and name and address of the establishment that prepared and packaged the food. In the front display counter observed cut melon and assorted fruits in to go containers without a label.
