Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Dishmachine tested at 10ppm chlorine. Chemical container was empty. Chemical replaced and retested at 50ppm chlorine. **Corrected On-Site**
12A-09-4High Priority – Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed employee with gloved hands place a raw beef patty on the grill then handle a clean utensil. Employee was coached on proper glove change/handwashing. Employee removed gloves, washed hands and removed utensil for cleaning. **Corrective Action Taken**
29-34-4High Priority – Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Outside back kitchen exit, threaded faucet with attached hose is missing a vacuum breaker.
