35A-02-7High Priority – Live, small flying insects found. Observed 1 live flying insect in dish area by triple sink. Also, observed 1 live flying insect in lobby by soda machine.
03A-02-5High Priority – Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. On cook line in reach in cooler, sliced American cheese (56F – Cold Holding). Manager stated it was pull from walk-in cooler and placed on line at 7 am. Sliced cheese was in placed in shallow plastic pan inside metal pan in reach in cooler. Manager removed cheese from reach in cooler and put in walk-in cooler to bring temperature down. **Corrective Action Taken**

36-73-4Basic – Floor, walls and/or ceilings soiled/has accumulation of debris. On cook line behind fryers has build up of grease and food debris. Also, at drive thru area behind soda machine floor has build up of debris. **Repeat Violation**
23-03-4Basic – Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Hood filters above fryers have build up of grease. Also, multiple gaskets soiled on cook line coolers. Also, around soda nozzles on soda machines at drive thru and in lobby have build up of old soda. **Repeat Violation**
22-16-4Basic – Reach-in cooler interior/shelves have accumulation of soil residues. Reach in cooler at end of cook line under where raw shelled eggs are stored has build up of mold-like substance on fan unit.
