14-86-1High Priority – Non-food grade paper/paper towel used as liner for food container. Observed towel used to covered bean sprouts in the walk in cooler.
08A-05-6High Priority – Raw animal food stored over/not properly separated from ready-to-eat food. Observed pooled eggs stored inside bread in the walk in cooler.
35A-05-4High Priority – Roach activity present as evidenced by live roaches found. Observed more than 25 live roaches crawling inside the oven of a stove located at the front cook line area of the kitchen. When oven door was opened roaches fell to the floor and started crawling all over the kitchen floor. Observed a live roach crawling inside the drawer of the reach in cooler where food is stored. Reach in cooler is under the grill in the cook line area. Also observed a live roach crawling inside clean utensils under preparation table in the kitchen area and another live roach in the dry storage under the sugar container.
