51-16-7Intermediate – – From initial inspection : Intermediate – No plan review submitted and approved – renovations were made or are in progress. Must submit plans and plan review application to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Plans must be submitted AND approved within 60 days. Plans on record are different : Storage area added with cooler and freezer , one of the bathrooms were removed , stove added to cook line , prep area added near walk in cooler. Operator agrees to lock the door separating bar from the food service establishment. – From follow-up inspection 2025-07-15: Per plan reviewer Rory , plans are in the process of being approved with additional information needed . Plan reviewer suggested a 30 day time extension due to plan review not being able to verify that deficiency letter was sent. **Time Extended** – From follow-up inspection 2025-10-27: Spoke with Cathy in plan Review. Plans were submitted by establishment 2-18-25 but were denied. A letter was sent to operator on 10-2-25 for the following reason: Lack of communication concerning the required information needed for plan review: 1) The plans as submitted are insufficient for plan review. Therefore we cannot perform an accurate review that will ensure there are no issues that arise at licensing inspection. Please provide a full site equipment plan reasonably to scale that clearly identifies all equipment including, but not limited to: handwash sinks, food preparation sinks, mopsink, warewashing equipment, cooking equipment, food preparation equipment, preparation tables, refrigerators, freezers, ice machines, ice bins, blender stations, wait service stations, service windows and bathrooms. 2) Please fully complete and return the attached construction finishes form for all areas of the establishment. No plans have been submitted after 2-28-25 **Admin Complaint**
