Observed 2 live roach on the floor mounted bread maker in kitchen, 3 live roaches on the wall behind bread maker, 2 live roaches crawling on the shelves where baking soda and seasonings are stored located across from cook line. **Admin Complaint** – From follow-up inspection 2025-08-21: At the time of the callback inspection observed 3 live roaches on the floor in the lobby, 1 live roach underneath the cambro at the front counter, 1 live roach by the bread mixer, 1 live roach on the floor by the store, 1 live roach on the floor by the reach in cooler, and one live roach on the wall by dry storage shelves. **Time Extended** – From follow-up inspection 2025-08-26: At the time of the callback inspection observed 3 live roaches under the prep table in the kitchen, 3 live roaches crawling on the chairs of the lobby, 1 live roach under the front counter. **Time Extended** – From follow-up inspection 2025-08-29: At the time of the callback inspection observed 1 live roach on the wall in the lobby. **Time Extended**
