Also, observed 1 live roach on wall behind stand up oven in kitchen. Also, 8 live roaches on wall under handwash sink in kitchen next to fryers. Also, observed 1 live roach on top of paper towel dispenser at handwash sink in kitchen near fryers, Also, observed 1 live roach on hand wash sign at handwash sink in kitchen next to fryers. **Warning** – From follow-up inspection 2025-09-30: At callback, observed 1 live roach under middle reach in cooler in kitchen. Operator killed. Also, observed 2 live roaches on middle reach in cooler in kitchen. Operator able to kill one. Also, observed 1 live roach on drain cover under handwash sink next to fryers. Operator killed. Also, observed 2 live roaches at front counter. Operator killed both. **Admin Complaint** **Corrective Action Taken** – From follow-up inspection 2025-10-01: At callback, observed 1 live roach on wall next to handwash sink in kitchen next to fryers. Also, observed 1 live roach on floor near fryers. Also, observed 7 live roaches under wooden pass through window in kitchen. Also, observed 2 live roaches in back of flip top reach in cooler motor area. Also, observed 1 live roach on handwash sink by grill. Also, observed 1 live roach under front counter. Also, observed 2 live roaches on wooden shelving unit at front counter. Operator was able to kill most of them. **Admin Complaint** **Corrective Action Taken** – From follow-up inspection 2025-10-02: At callback, observed 4 live roaches in gasket on left door of middle reach in cooler. Also, observed 4 live roaches in gasket on right door of flip top reach in cooler. Also, observed 9 live roaches on baseboard of wall under pass through window in kitchen. Also, observed 2 live roaches in gasket on top right drawer of cooler under grill. Also, observed 1 live roach on baseboard under 3 compartment sink. Also, observed 3 live roaches on wall right of ice machine. Also, observed 2 live roaches under baseboard in men's restroom. Also, observed 2 dead roaches on floor under handwash sink in kitchen next to fryers. Also, observed 5 dead roaches on edge around handwash sink next to fryers. **Warning** – From follow-up inspection 2025-09-30: At callback, observed 1 dead in plastic wrap cover ketchup bottles on front counter. Also, observed 1 dead under flat top grill. Also, observed 1 dead on wall right of flat top grill. Also, observed 2 dead roaches on floor in kitchen under pass through window. Also, observed 1 dead roach on outlet under pass through window in kitchen. Also, observed 6 dead roaches behind metal cover on back of reach in cooler next to fryers. Also, observed 2 dead roaches under storage racks in back dry storage room. Also, observed approximately 30 dead roaches on floor under front counter. Also, observed approximately 10 egg casings in back right corner of dry storage room. Also, observed 5 egg casings in motor area of flip top reach in cooler in kitchen. Employee began to kill up dead roaches. **Admin Complaint** **Corrective Action Taken** – From follow-up inspection 2025-10-01: At callback, observed 10 dead roaches inside reach in cooler next to fryers motor area. Also, observed 2 dead roaches inside reach top of middle reach in cooler in kitchen. Also, observed 3 dead roaches under middle reach in cooler in kitchen. Also, observed 2 dead roaches on floor by handwash sink next to grill. Also, observed 1 dead roaches in mop sink. Also, observed 2 dead roaches in threshold to dry storage room. Also, observed 1 dead roach under sink in dry storage room. Also, observed 1 dead roach under chemical rack in dry storage room. Also, observed 1 dead roach under white spice counter in dry storage. Also, observed 7 dead roaches under front counter. Also, observed 1 dead roach under reach in cooler at front counter. Also, observed 3 egg casings in motor area of reach in cooler in kitchen next to fryers. Also, observed 1 egg casing on floor in lobby. Also, observed 3 egg casing in motor area of flip top reach in cooler. Operator cleaned up and discarded most of the dead roaches. **Admin Complaint** **Corrective Action Taken** – From follow-up inspection 2025-10-02: At callback, observed 3 dead on floor in corner under flint top reach in cooler. Also, observed 1 dead under clean dish rack to left of 3 compartment sink. Also, observed 1 dead under dry storage rack in back room next to sink.
