Triple Sink (Quaternary 200ppm). **Corrective Action Taken**
03A-02-5High Priority – Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. On the cook line, butter (69F – Cold Holding). Operator stated butter has been out for less than an hour. Discussed time as a public health control with operator.

41-17-4Intermediate – Spray bottle containing toxic substance not labeled. Spray bottle is located at the bar, operator stated spray bottle is used to clean tables and counters.

36-32-5Basic – Ceiling shows damage or is in disrepair. There is a hole on the ceiling above the ice machine.
