Basic – Dead flying insects on premises. Observed at lobby of establishment along all windows approximately 25+ dead flying insects on window sills/ledge of interior windows. Observed at cook line hood filter system, approximately 15+ dead flying insects attached to hood system.

Basic – Equipment in poor repair. Observed low boy reach coolers at cook line unable to open/ hard to open. Observed gaskets at reach in coolers in disrepair. **Repeat Violation**

Basic – Floor soiled/has accumulation of debris. Observed at kitchen prep area and dry storage area behind shelves and fixed equipment. **Repeat Violation**

Basic – Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed gaskets at reach in coolers and freezers soiled. Observed counters and stove grids at cook line and under fixed equipment at cook line soiled with food remnants. **Repeat Violation**
