High Priority – Live, small flying insects found.Three flies found (2) in back near dish and drink area. (1) found in the bar area. Discussed the importance of this with manager. Contacted Lisa and discussed findings. Advised to educate operator.

Basic – Floor soiled/has accumulation of debris. Floor behind hot line had trash and excessive amounts of grease.

Basic – Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Electrical socket near induction burners had an excessive amount of rust present. There was also a slight build up of a soiled residue substance on the back side of the soda dispenser near the drink nozzles.

Basic – Reach-in cooler interior/shelves have accumulation of soil residues. Reach in coolers along the hot line were under the standards of shelving for a food contact surface. Excessive food debris and spills.

Basic – Reach-in cooler shelves with rust that has pitted the surface. Inside the cup cooling fridge the shelving had excessive pits and rust where the lips of drinks rest in the cooler.

Basic – Standing water in bottom of reach-in-cooler. There was standing water in the bottom of the reach in cooler in the bar area which had bottles of mixer for bar drinks resting in the puddle of water. Discussed with manager on duty.

Basic – Water draining onto floor surface. Standing water pooled up under the hand sink nearest to the dish machine area due to a leak from the hand wash sink draining to the floor around it. There was also water leaking from the hand was sink in the bar area creating standing water on the floor around it.

Basic – Water leaking from pipe and/or faucet/handle. Hand sink nearest to the dish machine area had an excessive leak from the drain as well as the hand wash sink located in the bar area.
