31B-02-4Intermediate – No paper towels or mechanical hand drying device provided at handwash sink. Hand wash sink back of kitchen near ice maker missing paper towels, operator went and got some. Dispensing device doesn't fit the towels on hand, new ones ordered. **Corrected On-Site**

16-55-4Basic – Dishmachine not washing/rinsing properly. Must wash, rinse and sanitize all dishware, equipment and utensils in three-compartment sink until dishmachine is functioning properly. Hopper for dish washer chlorine dispenser gasket is broken, parts on order and triple sink in use. **Corrective Action Taken**
36-01-4Basic – Floor not cleaned when the least amount of food is exposed. Floor soiled with syrup under drink machine for frozen drinks at the front counter.
