03A-02-5High Priority – Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Raw bacon in reach-in cooler on make line at 46F. Employee states they cooked bacon in the last few minutes. Bacon may have been left out of refrigeration for a few minutes. Bacon moved to walk-in freezer and approximately 10 minutes later, bacon is at 41F. **Corrective Action Taken**
29-42-4High Priority – Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. At mop sink by the washer, vacuum breaker missing from non-chemical side of attached splitter with attached green hose.

36-32-5Basic – Ceiling/ceiling tile shows damage or is in disrepair. Multiple ceiling tiles in the dining room show signs of water damage, some black-mold like substance observed on some tiles. 14-69-4Basic – Ice buildup in reach-in freezer and/or walk-in freezer. Ice buildup up observed in french fry reach-in freezer and the walk-in freezer at the fans.
