In the servers area there is water dripping from the ceiling onto the cutting board of the make table cooler. No foods have been observed being contaminated by the non potable water on the cutting board. Consulted operator and service staff not to use the cutting board or stage any foods on the make table cutting board. **Corrective Action Taken**

31B-06-4Intermediate – Soap dispenser at handwash sink not working/unable to dispense soap. The soap dispenser at the hand washing sink in the back kitchen, closest to the dish washer will not dispense soap.

36-37-5Basic – Ceiling not smooth, nonabsorbent and easily cleanable in food preparation, food storage, or warewashing areas. Damaged ceiling tiles in the dry storage area gave been replaced with ceiling tiles that are not smooth, non-porous, non-absorbent and easily cleanable.
36-34-5Basic – Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. There is a black mold like substance on the ceiling tiles throughout the dining area and entryway. There are water stains around the drop ceiling walls, as well as water dripping from the drop ceiling walls and air conditioning vents. There is a mold like substance around the air conditioning vents in dry storage. There is an open case dating to 7/17/2026. **Repeat Violation**
36-22-4Basic – Floor area(s) covered with standing water. There is standing water on the floor in the employee locker area.
08B-36-4Basic – Food stored in a location that is exposed to splash/dust. There is water dripping onto the cutting board of the make table cooler and splashing onto the closed roll top cover. No foods observed exposed to the splashing non potable water.
14-69-4Basic – Ice buildup in reach-in freezer and/or walk-in freezer. There is a buildup of ice in the walk in freezer around the door frame, on the floor under the fan housing and on the fan housing and pipes around the housing.
