Basic – Ceiling/ceiling tile shows damage or is in disrepair. Observed ceiling tiles in dining area and next to front counter with water damage and in disrepair due to condensation dripping from air conditioning vents.

Basic – Equipment located in an area not protected from overhead leakage of pipes, drains and other sources. Observed condensation, dripping from air-conditioning vent units in ceiling onto floor, a table in dining room and near end of front counter. Operator has wet floor signs placed in dining room, has moved table in dining room room to avoid drip, and move items on counter to avoid drip. **Corrective Action Taken**

Basic – Objectionable odors in bathroom or other areas of the establishment. Observed objectionable odor coming from men's restroom.

Basic – Observed multiple ceiling tiles missing in dining area.
