Basic – Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed black mold-like substance on the interior of lid and inside of the ice bin on the ceiling.

Basic – Ceiling tile missing. Observed one missing ceiling tile in the dining area.

Basic – Ceiling/ceiling tile shows damage or is in disrepair. Observed ceiling tile damaged in the following areas: Dry storage area, food preparation area, make line, drive thru area, behind front counter at expo line. Observed hole in ceiling tile to the right of fryers, above bag in box stand and to the right of ice machine. Work order submitted per manager.

Basic – Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed black mold-like substance in the following areas: inside ladder roof access, ceiling tiles and roof of dining area. Ceiling exhaust vents. Observed heavy rust buildup on ceiling exhaust vents.

Basic – Current Hotel and Restaurant license not displayed. Observed expired license displayed in the office.

Basic – Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed personal speaker on microwave in food preparation area.

Basic – Floor not cleaned when the least amount of food is exposed. Observed soda syrup on the floor under the bag in box rack. Observed standing water on the floor of kitchen area. Observed small wares under the food preparation table on the floor.
