03A-15-4High Priority – All potentially hazardous (time/temperature control for safety) foods in reach-in cooler cold held at greater than 41 degrees Fahrenheit. Observed all food in the True upright 2 door reach-in cooler is 50°F or higher. All food in this unit has been stored in this cooler overnight or longer. See stop sale for temperatures.
01B-07-4High Priority – Food with mold-like growth. See stop sale. Observed a gray mold-like substance on the top of a pepper sauce in a reach-in cooler.
35A-04-4High Priority – Rodent activity present as evidenced by rodent droppings found. Observed 15 moist dropping s on the floor under the prep table in the rear of the kitchen. Observed 10 moist droppings under the water heater and 2 large food storage bins in the kitchen. Observed 8 moist droppings under the flat grill on the cook's line. Observed 17 moist droppings in the cabinet behind the front counter. Observed approximately 20-25 on the floor next to the beer cooler behind the front counter. Unable to reach these droppings to determine if they are moist or dry. Operator began to clean up the droppings. **Repeat Violation**

14-77-5Intermediate – Reach-in cooler not maintained in good repair. Do not store potentially hazardous (time/temperature control for safety) food in this unit until the unit is repaired. Observed all food in the True upright reach-in cooler is above 50°F . Observed ambient thermometer for this reach-in cooler reads 49°F.

14-01-4Basic – Bowl or other container with no handle used to dispense food. Observed that plastic bowls are being used as scoops for cooked food stored in the reach-in cooler. **Repeat Violation**
23-06-4Basic – Build-up of food debris, dust or dirt on nonfood-contact surface. Observed a build-up of old food debris under a microwave oven on a prep table in the kitchen.
36-24-5Basic – Hole in or other damage to wall. Observed a new hole in the wall to the right of the rear door approximately 3 feet high and 9" wide.
