Observe cook crack eggs with gloves on then remove gloves and not wash hands. Addressed immediately and cook washed his hands. **Corrective Action Taken**
29-34-4High Priority – Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Vacuum breaker in wrong spot on hose. Person in charge placed in correct spot. Should be placed after the split on external hose. **Corrected On-Site**
