35A-02-6High Priority – Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed three small flies at entrance to kitchen not landing of food or food contact areas.
08A-05-6High Priority – Raw animal food stored over/not properly separated from ready-to-eat food. Raw shelled eggs stored above fresh herbs and coconut flakes. Operator moved food to proper storage. As a repeat violation the importance of following proper protocol with this violation was reinforced with the operator. 35A-06-4Basic – Accumulation of dead or trapped birds, insects, rodents, or other pests, in control devices. Three Mantis brand fly lights on walls in dish area, prep room and at end of expo line away from food with sticky paper full of trapped small flies. Operator replaced all sticky paper.
36-36-4Basic – Ceiling tile / vents missing. Ceiling vents missimg in dish area and on expo line.
33-24-4Basic – Garbage enclosure area in disrepair. Large commingled dumpster has large hole on left side of container.
33-16-4Basic – Open dumpster lid. Large dumpster located outside back door had lid open. Operator closed lid.
35B-03-4Basic – Outer openings not protected with self-closing doors. Rear doors not self closing and gap present at top left (looking from inside)
