03A-02-5High Priority – Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. raw beef, raw veggie burger (50F-58F – Cold Holding) in flip top reach-in cooler next to grill. Operator stated cooler was working fine earlier but thinks there may be an issue with outlet that it is plugged into. TCS foods were removed from cooler and placed in walk-in-cooler to chill. Also, sliced Swiss cheese, sliced pepper jack cheese (45F-46F – Cold Holding) in top of reach-in cooler next to hot dog warmer. Observed plastic tray in pan of cheese keeping cheese away from ice that was under pan. Operator removed sliced cheese to bottom of reach-in cooler to chill. **Corrective Action Taken** **Repeat Violation** **Admin Complaint**

22-02-4Intermediate – Food-contact surface soiled with food debris, mold-like substance or slime. Cutting board at flip top reach-in cooler next to grill soiled/ stained. Also, white cutting on clean storage shelf across from triple sink soiled/ stained.

22-20-5Basic – Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Back Interior of ice bin at drive thru window soiled.

Flip top Reach-in cooler next to grill unable to maintain temperature of 41F and below. Ambient temperature 59F. Operator stated cooler was working earlier today but thinks there may be an issue with the outlet that cooler is plugged into. Observed water dripping,from back fan area.
