Observed 25 flying insects on cart across from walk in cooler. Observed 1 flying insect on cook line. 03A-02-5High Priority – Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed in drawer cooler on cook line Beef (52F – Cold Holding); Raw chicken (52F – Cold Holding); sausage (51F – Cold Holding). Manager stated all meats were placed in cooler at 10am from the walk-in cooler. Cooler was not turned on at start of inspection. Manager turned cooler on and placed ice on TCS foods. **Corrective Action Taken** **Warning**

Observed commercially packaged salmon no longer frozen and removed from packaging in flip top cooler on cookline. Manager cut bags of salmon during inspection. **Corrective Action Taken**
