12A-16-4High Priority – Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands. Observed employee ( driver) start work did not wash hands before begin work, place pizza boxes in hot bag for delivery.

09-06-4Intermediate – Food employee applied hand antiseptic without washing hands as required by the Alternative Operating Procedure for bare hand contact with ready-to-eat food. No bare hand contact allowed with ready-to-eat food. Observed no hand sanitizer provided at all hand sinks throughout the kitchen, Reviewed Domino's handwashing procedures the Manager is not following the procedure. Manager sent employee to buy sanitizer for the restaurant.

22-08-4Basic – Interior of oven/microwave has accumulation of black substance/grease/food debris. Observed old dried food debris spilled interior of pizza oven and exterior. **Repeat Violation**
