12A-07-5High Priority – Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed the sushi chef prepare sushi rolls with raw fish, then she removed her gloves took the plate to the table and put on new gloves without washing hands and prepared a salad.
12A-17-4High Priority – Employee rubbed hands together for less than 10-15 seconds while washing hands. Observed the sushi chef washed her hands for approximately two seconds.
