32-09-4Basic – A minimum of one bathroom facility is not available for public use. Observed two bathrooms out of order. As per operator pipe connected to bathroom toilets is in disrepair. As per operator when toilets are flushed sewage water begins to back up from drains and toilets. No other available working bathrooms in establishment.
36-34-5Basic – Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed water stain on ceiling tile near walk in cooler.
